1. Use the links below to access the Federal and State Tax Withholding forms.

2. Submit (in-person or through the district mail) your completed Tax Withholding forms to your Payroll Specialist at the District Office. This form CANNOT be accepted by email.

Forms received after the 11th of the month may not take effect until the following month.

BUSD is NOT able to provide information or guidance on how to complete the tax withholding forms.
