In addition to the items listed above, you will also need to bring;
1. Original Certified (Official) Birth Certificate, Passport or Baptismal Certificate.
2. Original Proof of Immunizations.
3. Original Proof of Residency NO UTILITY BILLS.
4. Original Current Driver's License, California ID, Passport, Military ID, Other Picture ID
